We have updated our Wait List Form Please read all instructions before clicking the link below! Once you open the form, Fill it out by typing your information in the spaces provided. The form Can Not be e-mailed directly from this site. After you have completed the form, print it out and follow the directions to fax it, mail it or drop it off at our offices. You can also print the form and scan it to: housingapp@dcha1.org Then - if you would like confirmation, click on Options and “Request read Receipt”, Then send the e-mail. No other notice will be seen at the time you send the e-mail. Once your application is processed you will receive a letter of confirmation. You must report all changes and contact DCHA once a year to update. IF YOU ARE SELECTED FOR HOUSING A CRIMINAL BACKGROUND AND CREDIT CHECK WILL BE CONDUCTED TO SEE IF YOU MEET OUR HOUSING CRITERIA.
